 Pulmonary Rehabilitation

Dr. Bangayan - Imperial of Lincoln Park - Chicago, IllinoisUnder the medical leadership of Maribe Bangayan, M.D. Symphony of Lincoln Park has developed a pulmonary program designed to meet the needs of patients with chronic and acute respiratory illnesses.  Our post-hospital Pulmonary Rehabilitation Program is tailored to meet each individual’s needs and manages such illnesses as:

  • Chronic Obstructive Pulmonary Disease (COPD)
  • Asthma
  • Emphysema
  • Chronic Bronchitis
  • Pneumonia
  • Lung disease
  • Pulmonary hypertension

Monitored by a board certified pulmonologist, the Pulmonary Rehabilitation Program is designed to control symptoms, improve quality of life and return patients to their optimal level of functioning.  Our highly skilled inter-disciplinary team works together to create a personalized plan that addresses patients’ needs and monitors their day-to-day progress.  The program maximizes each patient’s independent functioning and prepares them for a safe return home.

 Cardiac Management Program

Dr. Sanjay Gill - Imperial of Lincoln Park - Chicago, IllinoisUnder the medical leadership of Sanjay Gill, M.D., our innovative Cardiac Management Program focuses on patients with coronary artery disease, congestive heart failure and those recuperating from post bypass surgery and heart attacks. This Program features:

  • Consultation by a Board Certified Cardiologist
  • Telemetry services available for heart rhythm management patients on inotropic medication
  • On-site Coumadin management, EKG, labs
  • Nurse and Nurse’s Aides trained in cardiac disease
  • LVAD (Left Ventricular Assist Device) management

Complex Wound Care

Dr. Maria - Imperial of Lincoln Park - Chicago, ILUnder the medical leadership of Frank Dimaria, M.D., our complex wound care program addresses the treatment and management of complex wounds including nutrition intervention, electronic stimulation, ultrasound, pulse lavage, MIST therapy and debridement.

Comfort Care Program
– Palliative Care

Dr. Hansberry - Imperial Of Lincoln Park - Chicago, Illinois At Symphony of Lincoln Park, we understand how difficult it can be to manage all of the issues that surround a serious or complex illness. Our Palliative Care Program, under the medical leadership of Maria Hansberry M.D., was designed to alleviate symptoms associated with life-limiting illnesses. The goal is to provide comfort care while meeting the emotional and spiritual needs of the patients and their family. Our skilled Palliative Care specialists work together with the physicians and the clinical team to provide options to enhance quality of life.

Infectious Disease Program

Our Infectious Disease Managemen Program, under the medical leadership of the Lakeshore Infectious Disease Associates including Toral Patel, M.D., James Sullivan, M.D. and Mitchell Weinstein, M.D., helps patients with an infectious disease or MDRO (multi drug resistant organism). Our on-site resources include:

  • Antibiotic management
  • Infection control protocols
  • Staff, patient, and family resources for infection control and management
  • Methods preventing unneecessary hospital readmissions
